DOH-Sarasota provides certified copies of birth certificates to eligible individuals. 

Order In-Person

For a certified copy of a birth certificate stop by one of our locations.  The cost for a birth certificate is $15.00 for the first certificate and $5.00 for each additional certificate purchased at the same time for the same individual.  A legal photo ID is required.

*Birth Certificates can also be obtained at the Venice and Downtown Sarasota Tax Collector’s offices. There is a service fee of $6.25 when obtaining certificate from Tax Collector’s Office.

To order by mail, send an application to the Sarasota office address listed above. A copy of a legal photo ID is required along with the application.
