Death Certificates

DOH-Sarasota provides certified copies of death certificates to eligible individuals. Death Certificates for deaths occurring from the year 2009 to present can be purchased at any county health department with a Vital Records office. 

Certificates for deaths occurring before 2009, must be purchased from the State Office, in Jacksonville.

Certified copies of a death certificate, can be requested at one of our locations. The cost for a death certificate is $10 per copy. A legal photo ID is required.

Order by Mail

To order by mail, send the completed application for death certificate to the Sarasota office. A copy of a legal photo ID is required along with the application.
