DOH-Sarasota provides certified copies of death certificates to eligible individuals.  Death Certificates for deaths occurring from the year 2009 to present can be purchased at any county health department with a Vital Records office.  Those occurring before the year 2009 must be purchased from the State Office in Jacksonville.  Below is the address and website for the State Office.

Order In-Person

For a certified copy of a death certificate, stop by one of our locations.  The cost for a death certificate is $10.00 per copy. A legal photo ID is required.

To order by mail, send an application to the Sarasota office address listed above. A copy of a legal photo ID is required along with the application.
