Enrollment fair on January 29 to meet deadline 

SARASOTA, FL – The last Health Insurance Enrollment Fair to meet the Jan. 31 deadline for coverage in 2016 will be held on Friday, Jan. 29 from 10 a.m.-3 p.m. at Waldemere Medical Plaza located at 1921 Waldemere St. in Sarasota. 

Certified Application Counselors and Navigators will be available to assist you with understanding your 2016 health coverage options. Reservations are strongly encouraged to ensure timely service. You can reserve a time by calling 941-961-8575.

When you apply for coverage or renew on the Health Insurance Marketplace, you will need to provide some information about you and your household; including income, any insurance you currently have, and some additional items.

Please bring the following to the Enrollment Fair:

• ID information for all members of your family (age, Social Security numbers, or legal permanent resident/green card or visa numbers).
  
  
• Income for all family members because you will have to estimate your 2016 family income to get your financial assistance (pay stub or tax forms, or other documents showing income.)
  
  
• Information about insurance offered through your job or your spouse’s job, even if you don’t take it, including how much it costs for a single plan.
